I recived a small check from CHI in the mail, wich had almost no infromation with it, exept an adress in Colorado, but it was mailed from Omaha for a bank in Pittsburg, the phone number to call for asistance is the number of this inqury, I suspect that if I cash or deposit it, I will be giving them my bank info, its not worth the $14, wich I have never payed to a Mercy facilty/provider, so Im shredding it.
